The online vendor application process makes it easy to complete all steps—form submission, logo upload, Health Permit upload, and a 25-word business description—directly on our website. Once submitted, our team will review your application and follow up with the next steps. No paperwork required!

2-Step Process:
Step 1: Download and complete the Contra Costa Health Permit. Then, return to the Digital Form.

For those who prefer a traditional option, you can download the Contra Costa Health Permit and vendor application from our website. Complete both forms and email them, along with your logo and 25-word business description, to:
